Have you ever revisited a design you did long ago and discovered the system no longer works as you intended? The author believes that, to help prevent this problem, every designer has a professional responsibility to create a succinct description of the basis of design, even in projects that don't use a commissioning authority. And, this basis of design should evolve and be corrected and amended through startup, functional testing and during subsequent renovations.
